Dignity in Every Emergency:
The UK’s first specialist-led emergency care planning service designed for those most at risk: care home residents, the housebound, and people living with dementia.
Because how we care for our most vulnerable defines us all.
From Pain to Purpose: A Doctor’s Promise to Families Like Mine
When I was in medical school, my wife and I lost her mum to a brain tumour.
Even with our training, we didn’t know what to do.
We had no plan. No knowledge of her wishes. No guidance.
We had to guess during the hardest time of her life.
That moment changed everything for us.
Now, after nearly 10 years working on the NHS frontline and with a rare mix of experience that fewer than 100 of the nearly 400,000 doctors in the UK have — I’ve built something new.
Something better.
To help families, carers, and patients avoid the same pain we went through.
What We Believe
At My Emergency Plan, we believe every vulnerable patient deserves three things in a crisis:
A Voice. Dignity. Protection.
We provide clear, expert doctor-led emergency care plans that make sure when something goes wrong, everyone knows what matters most and what to do next.
Think of someone you love or care for…Now ask yourself:
Do you know what their wishes are if something goes wrong?
Are you sure their carers, GP, or the ambulance team know?
Do they have a clear, expert-guided plan in place?
Most people don’t.
And the cost of that uncertainty can be heartbreaking.
Our Results So Far
Second cycle audit data: 100% of plans fully aligned with patient and family wishes
When patients and families are given specialist guidance and a proactive voice in their care it resulted in >70% hospital admission reduction (2-3x better than any existing model for hospital reductions)
100% of patients that passed, did so in comfortable, calm environments - Not in the hospital
What National Leaders Are Saying
“He brings an energy and a focus to advance care planning which I find really refreshing and energising… In our world of cynicism, Matt has just been a real light for me.”
— Professor Max Watson (MBE), introducing Dr Matthew Gloudeman at an internationally attended Hospice UK’s Clinical ECHO network
“You are asking us really important questions and raising really important issues… I encouraged you to go down more of a private approach because I think there is more potential there for maybe influencing. We thank you for your energy, we thank you for what you are doing and really appreciate your passion for this area.”
— Professor Max Watson, following Dr Gloudeman’s Project ECHO talk
“This is a much-needed service. Having worked alongside Dr Gloudeman as his Hospice Consultant and Medical Director, I know he will deliver this important work with both sensitivity and skill.”
— Dr Teresa Storr, Retired Consultant in Palliative Medicine with 45 years of NHS experience
